Our Focus: Improving the quality of our services and transforming how this care is delivered to improve health outcomes, patient satisfaction and the efficiency of the delivery of care. We want to dismantle barriers in order to achieve equity in healthcare.

 

Heartland Health Services Diversity, Equity and Inclusion

 

Through inclusion, education and empowerment, Heartland Health Services Diversity, Equity and Inclusion Advisory Committee fosters an equitable organization for staff, patients, families, vendors and community partners. Heartland Health policies, procedures and practices are conducive to developing workplace, patient culture and awareness that value Diversity, Equity and Inclusion. We acknowledge the many inequties within marginalized groups, and we are committed to dismantling barriers plaguing our communities. Memberships, Awards and Recognition

 

Heartland Health Services is a Federally Qualified Health Center (FQHC) and a proud member of the Illinois Primary Health Care Association (IPHCA) and National Association of Community Health Centers (NACHC).

The National Committe for Quality Assurance (NCQA) has recognized Heartland Health as a Patient-Centered Medical Home (PCMH). This program recognition reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP) and American Osteopathic Association (AOA) and others. The patient-centered medical home is a model of care that puts patients at the forefront of care. With this model of care, Heartland builds better relationships with our patients.

The Health Resources and Services Administration (HRSA) has recognized the high level of care Heartland Health Services provides for our patients by awarding us with the 2023 Health Center Quality Leader (HCQL) Gold Badge, an honor held by only the top 10% of the 1,500 FQHCs and look-alikes in the country. In addition to this badge, Heartland Health Services earned the following recognitions:

  • Advancing Health Information Technology (HIT) badge, awarded for utilizing technology services to reach patients.
  • Addressing Social Risk Factors badge for providing services to remove social determinants of health, such as transportation, interpretation services and patient education.
  • Community Health Quality Recognition badge recognizing health center programs (and look-alikes) that have made notable quality improvement achievements in the areas of access, quality, health equity and health information technology for the most recent UDS reporting period.
  • Patient-Centered Medical Home badge for being credentialed as a Patient-Centered Medical Home.
