Their Dreams start with sport and school physicals at Heartland Health Services.
The Illinois Department of Public Health requires students entering, transferring, or advancing into kindergarten, 6th and 9th grades to submit updated school physicals and vaccinations. Peoria Public School Disctrict 150 returns to class on Wednesday, August 2, 2023. Pekin Public School District 108 returns to class on Wednesday, August 23, 2023. Other area schools and school districts will need to check their 2023-2024 school calendar for back-to-school dates. Be sure to have your students forms up-to-date before school is back in session.
If your student plans on playing any sports they will also need a sports physical. In most cases, the sports physical form needs to be submitted before tryouts. Sports physicals can be completed at the same time as your students school physical. Just let your provider's office know when you schedule the appointment.
