Patient-Centered Medical Home (PCMH)


The National Committee for Quality Assurance (NCQA) has recognized Heartland Health Services as a Patient-Centered Medical Home (PCMH). This program recognition reflects the input of the American College of Physicians (ACP), American Academy of Family Physicians (AAFP), American Academy of Pediatrics (AAP), American Osteopathic Association (AOA), and others. The patient-centered medical home is a model of care that puts patients at the forefront of care. With this model of care, Heartland builds better relationships with our patients.

Community Health Center Quality Recognition (CHQR)

The Health Resources and Services Administration (HRSA) has recognized the high level of care Heartland Health Services provides for our patients by awarding us with the 2024 Health Center Quality Leader (HCQL) Silver Badge, an honor held by only the top 20% of the 1,500 FQHCs and look-alikes in the country. In addition to this badge, Heartland Health Services earned the following recognitions:

 

  • Improving Health Care Access badge for expanding services and making it easier for more patients to have access to the care they need, including medical and behavioral health.
  • Advancing Health Information Technology (HIT) badge, awarded for utilizing technology services to reach patients.
  • National Quality Leader badge for meeting high national standards in areas such as behavioral health, cancer screenings, diabetes, and heart health.
