Medicaid Innovation Collaborative

 

 

 

Thanks to the digital care resources of the Medicaid Innovation Collaborative (MIC), Heartland Health Services can send our Community Health Navigators (CHN) into the community to provide high-quality healthcare to some of our most vulnerable neighbors. Our CHNs provide virtual primary care appointments for five area homeless shelters, including the Dream Center, Salvation Army (both the family and men’s sides), Pathway Ministries, Esther House, and The Bridge. In their first year of operation, our team has accomplished the following:

  • Performed 581 telehealth visits
  • Scheduled 205 establishing care appointments
  • Scheduled 60 mental health appointments
  • Scheduled 113 ER follow-up appointments
  • Assisted with transportation, specialty referrals, and care coordination
