Our Services

Your Heartland Health Services team uses evidence-based care to keep you in your best health. Please let us know of any changes in your health, such as medications, hospital or specialist visits, or other medical services you receive. If you go to the hospital or emergency room, make sure to come and see us within 7 days. At Heartland, you're at the center of your own care.*

Heartland sees all patients regardless of their ability to pay. We offer a sliding pay scale as well as access to financial assistance forms and programs.

It is very important to come to your new patient appointment on your scheduled day and time. If you are not able to attend, please call and reschedule at least one day in advance. At Heartland, new patient appointments are scheduled for 30 minutes, allowing you time to get to know your healthcare clinician and discuss your health goals and plan of care together.
