If you are already established as a patient with Heartland Health Services, you can use your MyChart account to request a Primary Care appointment, view results, request medication refills, and message your care team.
If you don't already have a MyChart account, you can easily create one to access all these services. You will also be able to pay your bills online!

We can help coordinate transportation through your health coverage MCO, provide all-day bus passes through our partnership with Greater Peoria Mass Transit/CityLink, or arrange a quick pick-up and drop-off through Uber Health. Our team will work with you to determine the best option for your needs and assist with scheduling the service. We make getting here easier, so your health always comes first!
